The ß-BLOCKERS These drugs work by decreasing myocardial contractility, decreasing heart rate and reducing arterial blood pressure. They are discontinued to allow the patient to reach a maximum heart rate during the stress test.

The CALCIUM ANTAGONISTS

Calcium antagonists block the entry of calcium into cardiac and vascular smooth muscle cells. This causes a decrease in the heart rate and force of myocardial contraction. They are discontinued 24 hours prior to the test as their additional vasodilatory properties may camouflage areas of "at risk" myocardium.

ACE inhibitors should be discontinued for three days prior to commencement of captopril renal scans. .Angiotensin Converting Enzyme (ACE) is required to convert angiotensin(I) to angiotensin(II). Angiotensin(II) is a potent vasoconstrictor. Therefore, inhibiting the release of ACE interrupts the renin-angiotensin(I)-angiotensin(II) pathway with the net result being a drop in blood pressure.
